MY CHILD’S CARE:  A PARENT’S NOTEBOOK

AKA  “THE SNAPPY FINDER”

Dear Parent(s),

This notebook has been designed and made especially for you.  We have decided to give you this notebook because either your child has been recently diagnosed with a special health care need or your child has been living with a chronic illness.  In either case, you are not alone.  The notebook has been divided into sections with information sheets.  Please take time to look through each section and find those that will best help you care for your child.

Most hospital and physician offices maintain medical records that are incomplete, fragmented, and often not available when needed.  Emergency Room visits, hospital admissions and new physician or therapist consults usually have little or no background on your child.  This notebook was designed to help you maintain important information that is necessary for the family, other caregivers, and professionals involved to provide the highest quality care for your child.
